The survey sent under cover of this email is being conducted by the National Department of Health (Occupational Health and Safety Covid-19 Workstream) to assist in the planning for vaccination of workers (and possibly families and community members in the geographic area that you operate in). The survey is project managed by the Wits Health Consortium and supported by the DG Murray Trust and B4SA.

 

It is necessary to get data on the number of workers that could be vaccinated at work sites to facilitate the provision of sufficient vaccines as well as to identify which work places might be in a position to render vaccination services.

 

Please complete the survey (by clicking the weblink at the bottom of this letter) for the site of your operations.

 

Please also forward the email with this letter to each of the relevant people in each different site that your company / organisation has a presence at. Each of these colleagues must also please click on the weblink below to complete the survey for their particular site. This will facilitate the inclusion of all your workers as per their geographic locations across the country.

 

Associations, industry bodies or companies that have already submitted data to the National Department of Health or Business for South Africa are not required to complete the survey but can update any information (if they wish) through use of this survey instrument.
